The Columbia University TeenScreen Registry has been developed to support you - providing tools, resources and connections to colleagues who share your commitment.

The Registry places you on a national honor roll of the leading primary care providers offering mental health screening to adolescents.

The Registry is open to primary care providers that use validated questionnaires to screen adolescent patients for mental illness. To enroll in the Columbia University TeenScreen Registry, please sign up today.

Upon registration, you will also have the opportunity to join the Columbia TeenScreen Registry listserv and online provider forum.

As a Registry member, you will also receive a certificate and widget for your practice website and your profile information will be listed on the Honor Roll of Checkup Champions

Please note that Columbia University will not share or sell your information and that you will be asked to participate in brief electronic surveys no more than twice a year.
